  • This form is for students who are certain they do not plan to return for the following academic term in which they are currently enrolled. Completion of this form will result in removal from any classes in which preregistration has been processed. Understand that once you are discontinued, you must apply for readmission and contact the Admission Office for information on the readmit process.

I understand that offices will be notified of my intent and I will need to settle my affairs with the Student Accounts Office, Financial Aid Office and Student Life/Housing Office. I understand that failing to clear my account balance will result in my inability to obtain an official transcript.
